During this session, Mrs. Murphy reviewed the tax forms to enroll a participant as an employer, focusing on those forms that are always required and those that are sometimes required. The session reviews how forms can be completed for exceptional circumstances, with emphasis on streamlining operations and minimizing resource requirements. The presentation also reviews some miscellaneous forms, which can be used in certain ambiguous situations. The session was part of the 2011 FMS Conference.
